Steps: 1.)Right click your usb and go to PROPERTIES 2.)Go to TOOLS tab 3.)on Error checking click CHECK NOW 4.)Click the two CHECKBOX and wait for it top finish. 5.)you can now try to format your pen drive.
insert pen drive in safe mode. opern RUN from START menu. type : "FORMAT <drive letter>: /X /FS:NTFS" without quotes and press enter. drive letter is the pen drive.
Save the contents in the pen drive (this process will delete all the files in ur pen drive) and then download this http://www.apacer.com/en/support/downloads/Repair_v2.9.1.8A.zip,unzip it n run.....click "Format". Make sure u have connected only the pen drive u wanna recover to computer,this tool is very blind n it doesnt allow u to choose which removable drive to format..it formats the first removable disk (seems so). its probably a virus trying formating on windows 98 or on linux based system may work for u also check when the pen drive isnt connected properly or the usb port or connection is loose it may say a read only pen drive due to slow speed connection of pen drive to computer
