Well it doesn't block your card, it just blocks your sim card, If you will try a diffrent SIM in your phone that will work and if you want your sim to unlock than contact your Connection service provider means the company from you have you phone connection and you need to give them your sim no, which wouldb located at the back of your SIM and they will provide you with your PUK no for the SIM card.
3 unsuccessful tries locks the sim card,this is due the security code which you enabled for SIM card , you can still use it buy purchasing another sim card through the service provider that the phone was used with. Slip in the new sim card and it should work
A PUK (Personal Unblocking Key) is the code required for enabling/unlocking a sim card. This disabling/lock is a result of erroneously entering the PIN (Personal Identification Number) multiple times. Depending on the SIM card issuer, erroneously entering the PUK code multiple times would result to a permanently blocked/disabled SIM card. The PUK code is an OTW (one time write) only and therefore the only way to open the card is with the PUK code.
In all instances, it has nothing to do with the phone itself but with the SIM card, hence, the LG Vu CU920 would still work with another SIM card.

Hi your only option is to call your phone service provider, no it doesn't come with the phone and the phone never has its own puk code becouse the puk code is about the sim card.
if you put in another sim card the phone will work just fine, its the simcard 3 times pin code wrong puk code enabled pin blocked, call service provider to get it and entery instructions.
