Courtesy of Microsoft, here is the way to troubleshoot the problem with the 360 not reading discs:

Keep in mind, try other 360 games as well to determine whether its the game or a problem with the dvd drive itself.

My call of duty black ops disc is unreadable. Its clean and i restarted my console why is it not working?

There are a wide variety of issues that this could be, and seeing as how you didn't clarify which console your using, I'm going to assume its the XBOX 360 because this is a wide ranged issue with that console. If your only having a problem with that particular disk then I think it's safe to assume that there is only an issue with that disk. However, I'm going to direct you towards an FAQ about fixing unreadable disks on the XBOX 360 where you'll find troubleshooting tips and options on how to diagnose the issue with your console and fix the problem.

My ps3 wont read disk even after i've replaced the lens

If it is a certain disk that your Playstation wont read then its more then likely your disk is corrupt or unreadable. Perhaps its time to buy a new disk?
If your problem is that most or ALL your disks are unreadable then im sure the problem is to do with the playstation itself. (Proberly the lens again.) Contact Sony on their freephone and explain the problem to them. The default warranty is 1 years free cover. If you've had your playstation under a year then Sony will happily replace your console for free. Usually takes 2-3 working days.
Hope this helps!

Xbox 360 wont read discs

might be the games. any amount or size of scratches can make it unreadable, you can get them fixed in a electronic or music store, they scrape some of the plastic off the bottom and there you go if this was the problem, of its not you can always send in your console to microsoft and get it fixed/replaced.(you need the reciept to do this)

Xbox 360 disk drive not working right

Ok, if you are sure this disk is readable and all the other disk work fine, this will confirm optical lens damage. i would replace this optical lens assembly asap. this will fix your issue.

You should know that, if the Xbox states that the disk is unreadable, it may be the disk. if you are positive that this disk is good, simply replace the optical lens assembly.

Unreadible Disk as soon as I put the disk in.

do other games work? if not your magnet in your xbox console disk drive may be defective and i would suggest returning it to Microsoft to have them look at it. If other games do work your fallout 3 disk may be severely scratched or the magnetic strip on it may have gotten demagnetized or otherwise damaged and will most likely need to be replaced...sorry

Won't read disc's

theres a known problem with the hitashi dvd drive in the xbox 360.
the only solution to this is a home repair job, look up on youtube on how to do this, although this isnt recommended as it voids warranty.
so the best way is definitely just to ring microsoft and send ur console in for repair.
