I lost my manual. How can I get a replacement

The manual for the 5192 is not available even via Koss' web site.
The manual for the KS4192 should be very similar:

When we first start the unit, it has gotten quite a loud whirring sound. We turn it off, then turn it back on and the noise virtually disappears - it's still there, but greatly diminished. I suspect...

that is the fan on start-up and the high pitch is the power supply components heating up make shure that the main fan is working always it will be slow at low volume if it quits you run the risk of the power supply failing

Audio from tv

It sounds like you have the red, white rca cables hooked from the back of the tv to the red, white input on the stereo or the same connection from you hd sat box or hd cablebox. In order to get the same sound quality from tv service as you do on dvd is to run a fiber optic cable or digital coax audio cable from you hd sat box or hd cablebox to your stereo which means no red, white rca cables at all, take them out. Also once you do that you must be on a hd channel to get equal sound quality of a dvd movie. I know on some hd cable or hd sat boxes you have to go in and set them up to dolby digital as well after you hook it up the way I was explaining. Try this and I hope this is helpful.

Can TV Sound Come thru Sound System Speakers

if you have a digital rec the red and white output goes into the surround sound input, if you have basic cable, then you need to use the red and white output on your vcr to go into the surround sound input. just make sure to note which input you are using on the surround sound and switch it to that.

Will not play all of dvd-r

I have the same problem. The dvd-r's will play for a while, then all of a sudden won't. You have to do a factory reset, which resets all of your settings to the factory settings. To do so, you have to power the dvd player on without a disk inserted. Then press the menu key, then 76419. You may or may not have to hit enter. The dvd player will shut off automatically and when you power it back up, you should be able to play the dvd-r's, but you will have to reset you preferred sttings.

Koss DVD home stereo

It sounds more like the motor which moves the laser unit along it's rails is dry and sticking or maybe the rails themselves need a small amount of desintered grease applied. Any good hardware store should help with the grease. Just don't go using automobile type grease, it's far too heavy and will gum the works up. BMW
