there might be problem with the sim card holder. try to insert some piece of paper folded atop the sim card and close the sim holder

the sim card it came, i don't get why it needs the original one though.

As mentioned above you need to call the original network Telstra and O2 for the unlock codes for the phones, there are no free unlock codes available for DCT4+ handsets.

You can get the phone unlocked through most decent phone shops too, through flash patching using equipment attached to the handset...

two ways this can be resolved
1. have the unlocked either via direct flash or special code
2. purchase a TURBO SIMM AND THAT WIL allow you to use the phone cost 10pounds

If its just SIM locked, you will need to contact your service provider (ie Optus/Vodafone etc) and Request a PUK code. In alot of cases when you have PUK'd your phone, you will need a SIM Replacement, which you can get from the mobile Phone shops.

If you are trying to remove the network lock and have tried 3 times the phone is screwed and will need to be taken to a Nokia Care Center, they may charge.

It depends on which carrier the prepaid was with.
if The original prepaid and your wifes plan are both with the same carrier then it should be no problem just swap the sim over.
eg ( telstra to telstra ) or Optus To optus.
If the v3 is an Optus predaid and your wifes sim is a telsra plan then it wont accept the sim as the carriers lock the prepaid phones to their network
