Try this, go to xbox.com and download the update. Put it on a flash drive or burn it to a cd. Place it into the xbox and see if that installs the update.

you have to delete your online account.you can do this by going to account settings and then going to memory go to call of duty 4,modern warfare then click on your gamertag next to your gamer tag will be online progress do not press campaign progress unless you feel like doing your single player over.then press delete you will have to make a new account online but atleast you will be able to play.
Yes, this is because of the new Map Pack that was released, the game requires an update to allow them to work regardless of you buying the maps, because they had to update the playlist for it, if you do not accept the update you will be unable to play online.
It could be your router or network settings for your internet. Do you use Port forwarding? The Playstation site tells you all the ports you need to use. Also try putting your playstation in a DMZ.

I personally own the game and already had 4 patches. I would recommend you to download these patches as they were designed to improve some of the online aspects of the game.
As far as I know there is no way to delete the update process as it is automated as soon as your logged into playstation network (PSN)
You could always start the game without logging into PSN and connect to PSN as soon as the game started.
I can see no point in not downloading the updates and they are only 15MB so it should not take too long to download.
