I forgot my user mname and password for my wifi connection

You will have to either remember your password, or reset your wifi back to factory settings and set up a new password. In order to clear the "code 011" error, you will have to hold the reset button down on the Roku for at least 30 seconds to do a factory reset. But, don't do this until you get your wifi password issue squared away first.

If you forgot the d-link configuration password - the default user id is admin and password is admin
If that does not work then below the router there is a reset button use a paper clip to push and hold it down for 20 seconds it will reset the password to default ( NOTE : YOUR ROUTER CONFIGURATION WILL BE LOST IF YOU RESET THE ROUTER)
If you need the wifi password Click on the icon, which is usually next to the clock, and then right-click your connection. choose the wifi connection and right click right click and Select Properties and then check the Show Characters box. Doing this will display the password.

You need to enable WEP security on router. Connect router to 1 computer with lan cable then you can access it with below default ip in internet explorer. 192.168.1.1 or 192.168.0.1 or 192.168.2.1 then you can see user and password screen type user as "admin" and password as "admin" or blank or "password" or "1234" then go to wireless ->wireless security -> WEP note down the password . Which password do you mean? The wifi passowrd or the admin password?
The admin password is likely to be password (if it hasn't been changed) but the Wifi password you will only be able to see if you can access the router.
Easiest way is to use an ethernet cable between the router and the computer. Open up the netgear admin page (admin & password), then on the left hand side click on Set-up and then select wireless settings. Your wifi password will either be written on the next page or you will see four boxes of numbers with a blue spot against one of them. That one is your passowrd.
