This protects you from someone using your personal information or making calls if your cell phone is lost or stolen. You may see "PIN blocked" or Enter PUK code" on your cell phone display. The PUK code stands for PIN Unblocking Key. In the U.S., the PUK code is rarely provided when you get your subscription.
If you do not have your PUK code, contact the Customer Care customer again. Give them your phone number and once they are sure it is really you, they will give you the PUK code. You will then be asked to set a new PIN code. Once you have your PUK be sure to keep it in a safe place.
Note that if you enter a wrong PUK 10 times in a row, the SIM will be permanently blocked, and you'll need to purchase a new SIM card.
SIM card is blocked due to some reasons: -you`ve inputed wrong PIN numbers three times, the phone usually asks for PUK code - please note that you have 10 times chances input the code until it`s totally blocked by the provider -it was prepaid service and has passed the expiry date, you should change the SIM card in this case

The default security code for nokia is 12345 If you forgot you can generate master code using your IMEI, which can be viewed by pressing *#06# . If your handset asks for PUK code, you need to contact the service provider and provide them 20 digit sim number for the PUK code . Service provider numbers will be toll free. This code is OPERATOR specific and not phone specific and you have to call you operator and get PUk if PIN is blocked...
if you provide PUK code wrongly for 7 time, you sim may become useless, then you have to buy new SIM

Ask your network provider if they can give you its(SIM) PUK unlock code PUK1 and PUK2. Don't try to use the SIM lock for now until you have the PUK code. You have only three(3) try to give its correct PIN or else it will be totally block.
i think you blocked the pin code by entering the wrong pin code for 3 times so it is asking for the puk code ..
to get puk code you must follow these steps :
1. call to your network provider customer care
2. tell them that your pin code got blocked you want puk code
3. if they ask any details about the sim card owner you answer that one ...
4. then make a note of the puk code and enter in to your cell
