I know this sounds silly, but is your microphone plugged in?? Is it muted? Can you record anything using your microphone and Sound Recorder (Start, Programs, Accessories, Entertainment, Sound Recorder)?

If you have, for instance, Yahoo Messenger 10 India - it won't work because this version doesn't have any voice facility. Try to update your Yahoo Messenger version to the latest (Yahoo Messenger 10 US). If this won't work, contact Y! for help.
most likely with the headset you are using make sure it has headphones and mike built in
check if volume of pc is on mute check the ports behind the toer or pc you might connect them improperly no any other major issues try to borrow a working headset from a friend it might help you isolate the problem

2) Now Open the messenger with the same id n pass word u r using for Yahoo mail 3) On top of the yahoo messenger page click on "Messenger" 4) Scroll down and click on" Yahoo Chat" and Select " Join a Room" and click 5) On "Join Room " page Scroll down and select " Regional" and click and select the Country u want? Say " India" (if ur country is India or choose any country u like) and click on it and then all the rooms will appear on the right half of the page? 6) Select the Yahoo Public Room u want to be in? Say " Delhi Global Chat" and click on it? then the yahoo wl start to connect to the room and u must click " Enter Chat" box 7) Now u r near the room and here u wl be asked" To enter the chat room, please enter the characters shown above". U just type whatever excatly character is there in side the rectangular slot and click " Submit". This is usually a problem with bandwidth on her end or with improper microphone or sound card installation on her end. Sounds like your end is okay if you can chat with another person. You can have her click the Help menu in Messenger and select Audio Setup Wizard. Run through the setup wizard and be sure that all of her audio devices are set up correctly. If this doesn't cure the problem, then bandwidth is most likely the culprit.
It's clear that you are not using the correct version of Yahoo messenger, some version like Indian version doesn't support voice chat, but US version do. You can download the US version from this link http://messenger.yahoo.com/download/win/
But firstly you need to uninstall the installed version so that it'll not overwrite or retain the old version. Good luck.
You should remember, this is a fee service, so the quality is going to eb a little poor. Some of the things to increase, would be to eliminate all other softwares from running that would use the wifi or network connection. And have the person on the other end do the same. Of all the IM pc to pc chat/talk programs, Yahoo, seems to have the worst quality. if it is for voice only, I might suggest Gtalk, it seems to have the quality consistantly.
