You will get the PUK code from your cellular operator itself. So please contact the customer care of the cellular operator and they will provide you PUK code. They will ask for mobile no and the last paid bill or last recharge on your phone.

Your phone will ask for PUK code when you entered the PIN code wrongly several times depending upon the mobile make. The mobile asks for PUK code so that unauthorized persons do not use your mobile for wrong cause.
To get PUK code please contact your service provider through phone or go directly to their office. They will provide you the PUK code for your mobile. Try this -

1) Settings > Enable 3G Change to Off.

2) Use the iPhone GPS receiver to locate your position

3) Settings > Enable 3G change to On.
