Try cleaning it out first. Make sure there is not a disc logged in there sometimes when they fall it can cause the cd to become off track. Make sure if it is you might have to open it up and take it out some how.

Please make sure the file extension is MP4 format. Your psp working in this kind of format.
For your UMD also need to check your disc for correct format. If it is correct, you may have defective lens and need a replacement

It sounds like a typical loading error or an overheating problem, which would point to 3 main issues.
1. your disc is either dirty or scratched.
2. Your laser is out of alignment and needs to be tuned or replaced.
3. Your PS3 unit is overheating and causing the unit to freeze

Typically it is the disc that is at fault.

Try to troubleshoot by doing the following.

1. Try another disc. Preferably a new disc as then you can be absolutely sure that the disc is not scratched.
2. Try unplugging all the cables and then plug them back in again making sure they are all firm
3. Try putting a fan on your unit - keep it off carpet and in a well ventilated area. Fans work best.

If the first 3 options did not solve the issue you may need to send your unit in to get the laser aligned or replaced or the units internal fan checked.

First off, make sure the unit is not on hold. You can check that by looking at the side for the HOLD switch.

Next try to reset the player. Here's how you do it:
With the unit, get a paperclip and poke the reset hole at the back of the unit until the unit shuts off.

Try to turn on the unit and check if the buttons will be activated. If they'll work, format the player.

Next, what exactly happens when you try formatting the player using your PC?
You can try going to My Computer. There, you're going to see teh YP-K5. You DO NOT format that. Rather, double click on it. You're suppsoed to see a STORAGE folder. Right click on that directory and select FORMAT.

If Wii gives Disc Read Error, first step you need to check is to try another disc or clean the exisitng disc. This will confirm that problem is with either Wii or Disc. your os cd gone bad, try to get another os cd and boot the system from that cd.

I had the same problem, but I didn't have any valueable data on the DVD-RW (which was a Sony brand disc, go figure...). So, I backed up all the videos on the disc, tried to format the disc to get it ready to be able to be used again, and got the whole "Access Error" when I was formatting the disc. I tried both the VR and Video format options, the Video option came back with teh "Access Error" while the VR option came back with "Format Error". I put the disc into my DVD burner in my computer and was going to try and format it manually. But, as the disc popped up in My Computer and I right-clicked on the disc icon, there were no options to format the disc, which is unusual. I tried it in a laptop that's about a year old (versus the other computer which was about 4 years old), and as I right-clicked on the disc, an option was entitled "Erase this CD-RW". Obviously this was not a CD-RW, but I tried it anyways, and bam, after about half a minute, the mini DVD-RW was formatted! After that, I put it in the Handycam DCR DVD101 drive, formatted it using the Video setting (my preference), and now it works.

So, moral of the story is...try and format the disc manually on a DVD burning enabled computer. If it doesn't work, try a newer computer. But that's all I have, hope this helps!
