May be defective nebulizer, or may need diffuser reinstalled. Inside bottom of neb cup (unscrew top half) is small plastic piece (diffuser) that mounts directly over the hose connector (on outside) U can pull the plastic piece up to make sure its not too tight ,can come out-just push back on- or push down if too loose (poor air flow stream) If the diffuser is lost or broken you need a new nebulizer. If home care company wants to charge you more, tell them they gave you a defective nebulizer. Im a retired RRT
