We can't gave you that, only the network provider can do that, bring your sim-card to your network provider cuz' every sim-card has a unique number and that number is the access to your PUK code.

PUK CODE

all you need to do is just call custmer care from any other 02 phone and provide them with the SIM serail number, which is on the back side of your 02 SIM. they will give you the PUK code.

You should have a PUK number, on the plastic card where your sim card was.
Only your network service provider can give you that, if you lost the original.
