Radio Serial # (can be found in one of 3 places - white sticker in glove box, coin or ash tray and interior fuse box)
Copy of Driver License
Copy of Vehicle Registration

then call 1-800-382-2238 - option 5

Provide them with that information, they will then give you a fax number to send a copy of your registration and driver license to and they will call you with in 24 hours with your code.
