Problem > Engine surges between 1500 & 2000 revs after initial warm up. Check for codes - Nil. Cleaned throttle body & MAF. Checked for any vacuum leaks - Nil.

you should change the tps( throtal position sensor) this can cause that problem

Try changing your PCV valve make sure the hose is clean and not full of oil. Also check the elbow end that goes to the intake they tend to collapse from oil saturation. Hope this helps.

SOURCE: code 23 engine idles but surges when in gear

Replace the Intake Air Temperature Sensor.
Depending on your size Engine (2.4L, 3.0L or 3.3L) It should be located where the Air Filter is. When you take the cover off, it wil be mounted into the cover. It has the only wires going to the cover.
This sends information to the Computer which adjust the air/fuel mixture - causing surges.
The 2nd recommednation: to remove, clean and check is the Idle Air Control (IAC) Valve and replace if necessary.

Check for vacuum leaks with a can of carb cleaner engine running - when engine race up or bogs down there is a leak. Spray all around intake, vacuum hoses, brake booster, and throttle body.

Nissan x-trail diesel code p0089

I would need to know what year model this is, engine capacity and engine code to confirm but generally on the x-trails that fault code refers to a mechanical fault on the fuel pressure regulator. This is for the YD22DDTi engine.

94 tauras AT once warmed up trans will drop out gear at a stop

This is a common problem. You have a cracked forward clutch piston. Ford made this part out of aluminum. The replacements are steel. But you have to disassemble the transmission to install it. ( I know you said Rev too, In the AXODE trans, the forward clutch is on in 1st and reverse)
