PUK & PIN codes are not related to the Phone. These codes are Security codes on the SIM. Get in touch with your Network Provider. they are the one who know your PUK & PIN codes
To get the Puk number of the line you are using, you need to contact the service provider of the line you are using (customer care line) to be able to unblock the puk and retrieve the line back.

the puk code is supplied by your provider after you have entered the wrong sim card pass code more than 5 time. if i am understanding this correctly you actually need a PUK code. You can either go to your provider store near you and they will unlock your sim or you can go onto their website and get a puk code, but not all providers offer the puk code over the net.
The puk code is sim card spesific it comes with the sim card when you get it or you can get it by calling your phone service provider of the sim card to get the puk code.
