Dont use 1920x1080. Its probably being very heavy for your graphics card. You havent specified what graphics capacities your PC has so I cannot tell for sure. But I play COD MW using HDMI cable on a 1080p TV too and what has worked for me is,

Set the TV resolution to 1280x800 and the same for COD MW. This is the best and safest widecreen option. You can give 1400x900 a try but I am pretty sure 1280x800 will work fine.

Attempt to clear the system cache to see if that helps. To do that hold down the sync button (little white button right beside the second Memory unit slot) and then turn the console on. Continue to hold the sync button until the 360 loads fully to the dashboard. This should clear any failures in the system.

Try using a good quality dvd cleaner first. A dirty lens is usually the problem. Also check your game discs for any scratches as scratches also can cause this problem. Good luck and hope this helps.

Sounds like a problem with the disc. You can take it into most game retailers and get the disc repaired.
