The PUK code is a code associated with the SIM card and not with the phone.
You have only two ways to get rid of the PUK code. Either you replace
the SIM card , or you call phone provider that issued the sim cadr (eg.
At&T, Virgin, Tmobile , Vodafone, Telecom etc.)
PUK means "personal unlocking key" .
The PUK code is asked if you enter a wrong pin 3 times.
You need to call the phone company that released the SIM card, they can retrieve the PUK code from a database for free.
In your case you need to contact your SIM card provider technical support to unlock the SIM.

This could be due to a faulty sim card or a software problem. Check to see if the sim card is not damaged. Try it in another phone or try another sim in your W850. If there is nothing wrong with the sim then you can try a master reset or software flash of the phone (back up everything since this erases all data).
