Sounds like you have a defect LCD panel tab !!! These are also common with Samsung LCD's as well. The only permanent fix is the replace the panel which will cost as much as a new TV in most cases.
a BLACK cloud if covers the entire screen and even looks like motion swirling but very dark screen could be a bad board OR may be the front display Panel.
On PLASMA SETS IT IS A PLASMA DISPLAY PANEL(pdp) OR on LCD sets Ldp ----in either case it is a sealed unit with an inert gas inside--if the panel ruptures inside you will have exactly what you describe.
Sometimes there is a defect in making this unit--sometimes if set is transported even a mild jolt will do this.
Bottom line is if the display panel is bad--fatal from a cost view as the part alone will cost more than the set itself.

Just check whether there is any trace of picture [will be ver dark indeed]; can be seen on screen. Do a close watch and inform me. As far as sound is OK, there must be a dark shadow of picture on screen. I can post a solution to your tVs trouble only after get this information. OK.
Is tv plasma or lcd (how old). it may be the screen or pannel has lost its efficiency. A bit like old tv's tube going. You did the correct thing by adjusting controls but this sounds like a workshop job
Do you get any display up at all? Menu, OSD? Is the PDP priming (lighting up) The best way of testing this is making the room as dark as you can and seeing if the screen is lit.
If there there is none of this then you either have faulty Scan boards or the PDP panel has blown.
Go into the customer menu and find the "closed captions" heading. Within the closed caption you will find "TEXT". If you turn off closed captions or the text, whichever you menu lets you do, your rectangle will go away. Hope this helps
