Verify that there is nothing clamped between the platen and the shuttle.

Last time I had this fault, it turned out to be a broken wire in the harness that connects the CMX board to the shuttle module.

Other possibilities include a failed shuttle or CMX board.

You have probably tried to reflash the printer with a firmware level that the printer does not support for that Security PAL. Printronix requires a specific Security PAL to 'unlock' the firmware so you can use the printer. I would get the original firmware for the printer or purchase the upgrade if you are trying to get some specific option to work.
