I would suggest that you have it serviced. It definitely sounds like the timing is off, but adjusting the timing can be tricky, and if it's not done properly you can REALLY screw things up!
Sounds like it's advanced.... but take it in and have it done professionally... have him lube and clean and check for cracked parts while you are at it.
Yep, it's out of time. Take it in for service. Ask for cleaning, lubing and re-timing. Otherwise they will just time it and if it is dirty and dry... it may just fall out of time again.
Also remember you can never turn the hand wheel backwards to the way it is intended to go... that causes this issue in some machines.

Check you have the right size bobbin case the gap where the needle enters should be 13mm approx at the widest point. A lot of other pfaff bobbin cases seem to fit and can be easily be mistaken. I have two pfaffs and have accidentally mixed the cases and had a similar problem.
A bent needle will hit the metal. If it continues you will need to adjust the needle carrier. hopefully that isn't bent, but if so I don't believe that the replacement of it will be too costly.
