You have to get a new SIM card from your provider.

what happened is that you locked your sim card out of your phone, You can either contact att for them to give you the puk code or you will need to get a new att sim card.

you have entered the pin code wrong 3 times now you are required to enter a puk code to reset the pin code. 
imei has noting todo with puk code which is on the sim card which is not related to the phone at all.


The puk code is sim card spesific it comes with the sim card when you get it or you can get it by calling your phone service provider of the sim card to get the puk code.
