You could ask your provider to tell you the puk code. The puk came with the sim card. Same paper or same card where the first pin was on too.

It's not the problem with your phone, contact the customer care of your service provider (network operator), and tell them your problem, they will give you the number instantly. If you enter wrong/random numbers several times, your sim card will be unusable. Good luck
