Hi, Uninstall all the software and once this is done, power on the scanner, remove the USB cable and start a new setup installation. Let the program tell you when to re-connect the USB cable to the computer. Let it finish and reboot computer to complete the installation.
Go in control panel and click on Scanners and Cameras and you will see the HP icon there. Click it to activate the scanner and a menu will pop up. Scan from that menu.
That's a laser/scanner error. Get to the DIAGNOSTICS menu. Run the laser/scanner motor component test. Listen as the motor rotates. Call HP. your printer is under warranty. Tell them if you heard the motor noise.

open the scanner place the document face down on the glass, go to the control panel on the printer and press scan , this one method, the other is open the click on start menu, move up to accessories, after that move to scanner etc, and you are done
The printer will come with scanner software or you can use the Windows scanner program on the Accessoried menu. It will say Scanner and Camera Wizard. You need to load the drivers first.

You say that the scanner glass is clean, but it takes next to nothing to cause these lines. Couldn't hurt to chack again (if you havent cleaned it 6 times already).....anyway, I found this document that is not specifically for your machine, but the menu commands should be the same: Recalibrate the product scanner:
Press ENTER/MENU on the control panel of the product.
Press the < arrow key once to display Service and press the ENTER/MENU button.
Use the < and > keys to select Scan Correction and press the ENTER/MENU button.
Insert a blank, bright white piece
of letter (8.5 x 11 inches or 216 x 279 mm) or A4 (8.3 x 11.7 inches or
210 x 297 mm) paper into the document feeder tray.
