WHAT DO I DO WITH THE PUK CODE?
Once you have got the PUK code from your network, you will need to enter it into your phone. A notification will usually pop up, prompting you to enter the PUK code as soon as you turn your mobile on, so just follow the instructions onscreen to proceed.
As soon as your phone is unlocked, you may be asked to enter a new SIM PIN number, but this can be turned off in the settings menu of the phone once it's fully unlocked.
WARNING!
Do not try and guess your PUK code. If you enter it wrong 10 times, the SIM card will be permanently locked and you will need to get a new one from your network provider. There may be a charge for a new SIM card and you might be stuck without your phone for a couple of days while they send it out.

Puk can be given by your Service provider, It has nothing to do with your phone code or phone lock,Just the Sim card.

You can contact you Sim service department for help by calling. Call is free.You might not be able to contact them through your cell phone ,cause your cell phone is not accessible yet until the Puk code has been removed. You can use afriends phone that uses the same Line of Sim card as yours.

Go to the nearest service outlet of your service provider they can also help you.

The puk code for telephone is the special code that allows you to do a master reset on the telephone. You will also need that number to unlock the telephone after it has been locked.

Your original SIM card package would have come with the PUK code.

If you cannot find the Provider Unlock Kode, then just ring your mobile provider Help Desk and request the code from them for your SIM card and phone number.

If you manage your mobile online, normally for Virgin Mobile, the PUK code is often available in the User facility pages.

When you enter a wrong Pin code three times, the phone goes in lock mode, and ask you for the puk code.

The puk code was enclosed with the original sim card documentation.

If you lost it the only ways to recover the PUk code are either paying a phone shop (they will phone virgin), or calling virgin mobile customer service by yourself. They will ask your personal details, and retrieve the PUK code from database.
