this sounds strange, but is true. you need to run your digital audio optical cable from your tv to your receiver (after, of course, connecting tv/receiver via hdmi). so, hdmi from blue ray to rec - hdmi from rec to tv - then optical from tv back to rec. it's round about, but that is how yamaha says (and the only way it works) to do it.

I had the same problem. You should check to make sure that the switch on the XBox cable is set to HDTV and that you change the resolution on the XBox to 720P/1080i.

the trick is, it has NOTHING to do with your receiver, and EVERYTHING to do with your TV, you need to choose SAP on the TV not the stereo. That will send the SAP sound to the stereo you are playing the sound through.
