As I understand from personal experience, the PUK code is only available from the service provider. You need PUK code after you entered the wrong PIN 3 times for the SIM card. Call your service provider or get a new SIM card.

you will have to contact att and ask them for an sim unlock code or have the device your using unlocked and jailbroken, if its a tmobile device they will not give you a unlock code you will have to have an outside source do it for u, good luck

PUK stands for Personal Unblocking Key. This is a unique eight-digit code specific to your SIM card. This is required when the PIN number to unlock your mobile phone is entered incorrectly 3 times. the galaxy tab will be locked to the provider you got it from, which is why your sim isnt being recognised. You will need to unlock the tab to allow it to accept any sim. To get it unlocked you need to request an unlock code from Telstra
