I need to unlock it

My phone said exactly the following:

FCC Id: BEJC1300i
IMEI: 010564-00-412742-2
S/N: 512cqsf412742-FID-P40

it is locked to Fido - Canada.
i get to teh point of 2945#*70001#, but i need the restriction code.

    My LG 1300i requested a PUK code when I set up a PIN code feature in security. I had 8 tries to figure out what this was and get it right. I failed. Now my SIM card is also locked. What is the solution to this? What is the PUK code number? How do I get use of the phone back. I have a prepaid plan with AT&T, unused minutes and data.
