An MLX phone plugs into ports on a Legend or Magix phone system made by Avaya/Lucent. If you don't have either one of those, the phone is a paperweight.
The CID comes from the trunk card and cabinet. It's probably a Legend or a Magix system. If you don't have the proper hardware, you need to order and program it.
Your best bet is to call your AT&T/Avaya/Lucent installer and check with them as it's beyond the scope of a free forum to give exact instructions.
