After you click on the desktop app and can see your desktop (don't confuse it with the apps background, you can't change that), right click on it, chose "personalize" then in the window that comes up you'll see "Desktop Background" at the bottom and click it.
From there you can use the "Browse" button on the top to find images you want to use. You can choose multiple images to load and set how often they can change.

Have there been any changes to the Start Screen in the new Windows Phone 8 version?

Yes - Microsoft have actually made quite a lot of noticeable changes to the start screen in the new WP8, such as;

- They've kept the live tiles but they now have support for 3 sizes which lets you customize your screen much more.

- You can choose from a pallet of 20 colors to customize your screen.

- There is now multiple screen sizes and a range of resolutions to choose from.

- You can change your lockscreen background and even choose which apps you want to be on the background.
