The adapter has to be defective. Theres just no other explanation. if you look at the plug that goes into the ipod is there 2 black circles? or just one.
Which model of bluetooth headset you are using?
If you have another ipod or know someone who does test them in that. If the problem is not there then the port on your ipod is malfunctioning and needs to be repaired. If it does have the same problem then the adapter may be mono and you need a stereo adapter.

The adaptor is specifically for the ipod. Is there way of getting sound in both ears?
