The PUK code is a code associated with the SIM card and not with the phone.
You have only two ways to get rid of the PUK code. Either you replace
the SIM card , or you call phone provider that issued the sim cadr (eg.
At&T, Virgin, Tmobile , Vodafone, Telecom etc.)
PUK means "personal unlocking key" .
The PUK code is asked if you enter a wrong pin 3 times.
You need to call the phone company that released the SIM card, they can retrieve the PUK code from a database for free.
In your case you need to contact your SIM card provider technical support to unlock the SIM.
The PIN and PUK are related to the SIM card, not to the phone.
If you replace the SIM card with a different one, then the phone will not ask for the PUK anymore.
NOTE:
Entering the incorrect code more than 10 times will cause the SIM card
to be permanently locked and will require purchase of a replacement SIM
card. To buy a new SIM, please visit a store
