If your lucky enough to be in the UK and with O2 Uk? You can go to your nearest O2 Store who can arrange a sim replacement for you, keep your original number, keep any remaining credit (if pay & go) and usually its free!
teddc is right with the sim data though, as the sim locks itself permanently to protect data or being misused by someone unauthorised!
Sorry my friend, but you will have to purchase a new SIM card. As a security feature, once the PUK is incorrectly entered too many times it "Fries" the SIM. You may be able to talk your provider into giving you one, but the only solution here is a new card. Any information you had on it is gone. SIM cards usually run around $20 to replace.
