depends. You modem must have a serial number. If you know your brand of you modem like alcatel, siemens or linksys etc.etc. you can try to make a fake online warranty request.
Just go to the website of your brand. look for support / warranty. And you get your model.
Or.... easier look under the modem on the sticker if it say's p/n it means productnumber and usually this is your model.
