Two products I recommend for cleaning and repairing CD's and DVD's are Windex and Brasso.
Windex will clean the disc very well. Brasso will polish the disc surface and remove scratches. Use a felt or cotton cloth, an old t-shirt should be fine. Lay the disc flat on a bench on top of an old tea towel. Keep wetting the polishing cloth up with Brasso and give the disc a severe polish. Wash off the Brasso under the tap and dry the disc then clean it with Windex.
Final step - Make a backup of the disc.
I tried toothpaste like some people recommend, it didn't work. I tried Jif and it made things worse. Brasso is the answer. $8 at the local Supermarket and you get enough of the stuff to repair about 50 discs.
If it is not seriously scratched you do have a chance to save it . Go to your local gaming store and ask for a disk fixer or disk repair kit which can cost about $8 for a cheap one or about $30 for a good one. The good thing about these fixers or repair kits is it can also be used on DVDs as well. Hope I helped.

the disk seems scratched (not too badly) it loads to the main menu of fallout 3 but when i click continue it says unable to read disk
If the software is seriously damaged, you will need to but another copy. the lens will not be able to read pass a major surface scratches.
