That means that the phone charger is sending an incorrect voltage to
the phone, or that there had been a contact on phone charger connector.
You can temporarily restart charging by unplugging and plugging back the charger, but on the long run this can damage the phone.
To start check the connector at the base of the phone, where you hook up the charger.
If there is dirt or oxide in there remove it, you can clean it using cotton buds and isopropyl alcohol.
If that does not solve the problem, go to any phone shop and buy a replacement AC charger for $10 or less.
