The lock code is from your previous provider. They have apparently locked your phone into there company. You can ask them for the code but you will probably have to pay a small fee to release your phone. Or you can go online and find the code if you know how to look for it. Ebay sells codes to release your phone as well if that is any help to ya.
Hope this helps and good luck.

you will have to "block" your card by putting in the wrong password 3 times, you will then be asked for a puk code, when putting in your puk code, you will be asked to put in a new pin number. ask your service provider for your puk code.
Please try these codes. This might work fine for any sim...

hello there....well its quite simple...there is nothing wrong with your phone ...its your SIM... if it is asking for a PUK (Personal Unblocking Key) code...call your subscriber and ask them for PUK code, entering this code will reset your sim
If the error message is saying sim damaged call them and ask for a "sim replacement" you will get a new sim while you will retain the same number ,however you will lose the phone numbers and other data stored in sim, this can be free but it depends on the company and how 'customer friendly they are
Open phone, don't go to any menus. Dial *#7780# and press green send button. It will say if you would like to restore factory settings and say yes. Then it asks for a code. This is when you put 12345 and then press OK. This should help because it did for me.
