On 7/4/13, a unauthorized payment was taken from my checking account. This transaction was supposed to be stopped approximately 4 or 5 months ago. I am suspecting fraud. I do not have auto pay on my Directv account.USBank says they cannot remove this action and to notify Charge Smart

You should contact the bank immediately if you want your money back.
Log onto your payal account open up a dispute in resolution tab you will have to find this tranzaction and copy the details down date / itemno / etc...(write them down) make a list of problems you have with the seller to make a claim.

Before you start please check the status of the transaction in question. If the transaction states that the payment is unclaimed or pending then you should still be able to cancel it. If it does not have a status of unclaimed or pending then you should contact the payment recipient, explain what happened and ask for a refund.
To cancel an unclaimed payment sign in and click the My Account tab.
Click the History tab in the sub categories section.
Click Cancel link in the Action column of the transaction in question.
Click the Cancel Payment option.
You should receive a message that says that you have successfully canceled the payment.
