Hi!you can find your simcard PUK code at the back of the holder of your simcard when you buy it..You can find there also the initial PIN code of your simcard.
Remove the SIM card from your phone, clean the contact point (the copper part) of the SIMCARD with a rubber eraser then insert the SIM card back again to your phone.
If SIM card is still failing, then it is already defective. Get a new SIM card.
The original SIM has all your account information stored on it. The new SIM card must be activated either by transferring your your number to it or starting a new line. The only way I know of to do this is to call customer care from a landline with your SIM id. number and the phone number and name of the account holder.
is this happens to other sim you inserted on your phone...if still occurs it seems you have a loose simcard socket..or the copper contact itself miss a connection... try to clean your sim esp. the copper surface also do the same in the simcard slot.. put your sim and gently apply force or put a ply of paper on top of the sim and close the phone cover. turn ON your phone and check if the sim already register....nice day

If the problem is still not finished, then you need to change sim ic new.

if you can not see a simcard holder then the phone is not GSM but CDMA which does not work on simcard use.......but have someone check the phone for the card first.
Remove the back housing on the bottom half of the phone if the battery is in there then remove the battery aswell
then at the bottom of the area where the battery sat there should be a small metal plate with 6 or so little copper nobs underneath it.
slide the sim between the plate and the copper nobs making sure the cut off corner of the sim locates in the blanked corner of the slot.
Also make sure the copper face of the sim is what will be touching the copper nobs
