If you forget your 4-digit code
If you forget your password, to clear the current pass-
word, follow the procedure below.
1 Press SETUP to display the setup menu.
2 Use the Number buttons to enter the 6-digit number
"210499".
The 4-digit password is cleared
If you forget your unlock code: At the Enter Unlock Code prompt, try entering 1234 or the last four digits of your phone number. If that doesn't work, press - and enter your 6-digit security code instead.
NOTE: Your phone's 4-digit unlock code is originally set to 1234. The 6-digit security code is originally set to 000000.
Your phone's 4-digit unlock code is originally set to the last 4 digits of your phone number, and the 6-digit security code is originally set to 000000. Your service provider may reset these codes before you receive your phone. If your service provider has not reset these codes, we recommend that you change them to prevent others from accessing your personal information. The unlock code must contain 4 digits, and the security code must contain 6 digits.
If you forget your unlock code, try entering 1234 or the last 4 digits of your phone number. If that does not work, press the menu key, in most of the phones this will bypass the unlock code screen, and the phone will now be asking for the security code. If you forget the security code, send your unit in for repair to a level three LSC. There could be a charge for this as it is not covered under warranty. To change a code or password:
Press the Menu key
Use the Navigation Key to scroll to Settings, and select this option
Scroll to Security and select this option
Scroll to New Passwords and select this option
Select the type of code you wish to change
Enter the old code when prompted, then enter the new code

You can lock your phone to keep others from using it.
To lock or unlock your phone, you need the four-digit
unlock code.
To manually lock your phone: Presss>
wSettings > Security > Phone Lock > Lock Now, then enter
your four-digit unlock code.
To automatically lock your phone whenever you
turn it off: Presss>wSettings > Security > Phone Lock >
Automatic Lock > On, then enter your four-digit unlock
code.
According to the user guide which you can read at the link below, the factory-set USER lock code is 1234. The document also covers lost codes, saying, "If You Forget a Code, PIN, or Password Note: Your phone's 4-digit unlock code is originally set to 1234, and the 6-digit security code is originally set to 000000.
