Toshiba Satellite A135-2356 has non-functioning USB ports (all of them)

Device not Recognized is the message
Device Manager states that all USB controllers/devices are functioning properly
By-passed the problem by using a PCMCIA-USB card, but the slot does not seem to provide enough power to operate an external portable drive

I just started having almost same exact problem.
A few weeks ago my system locked up hard and I had to hold power button down to reset. This caused a password to set itself on the BIOS. The system could no longer be started without the password. The only way to reset was to remove memory and jump a pin on the board.
Anyway, after this episode I found that BIOS v1.4 corrected this issue.
I installed BIOS v1.4 at this time. Then a few days ago the USB ports started acting up.
I just found BIOS v1.6 and installed it. Problem still exists.
I have been using 2 of the USB ports full time since I bought the system. One for mouse and one for external hard drive.
Never had any issues for past 2 years.
I will see if I can get some free info from Toshiba Tech.
Hopefully we can find a solution for this.
After the BIOS password experience nothing surprises me anymore.

Have you used this device with this laptop before? If not, there may be specific USB drivers required to operate it that must be installed prior to plugging in the device (hence, the 'Device not recognized' message).
What operateing system do you have installed?
what is the model of the PC card?
How often did you use (take in/out USB devices), all of them?
