Try it without the mods (If there are unknown variables these fixes might not work). I had many problems with my fallout 3 (I am a bethesda fanboy so I stuck to the game despite all the problems). The first fix i reccomend is going to your system settings and then to your memory and then clearing its cache (will not affect your saved games). The second fix I recommend is deleting the patches and letting them download again. The third fix is the corrupted data deletion. Sometimes changing options in the options menu created corrupted data that would show up somewhere in your fallout game saves folder (labelled corrupted data). Sometimes this corrupted data can be linked to a specific game save (deleting a game save is a worse case scenario).
)The forth fix (is true for most every bethesda game) is limiting your saves (for me i only have 2 at any given time and never anymore) seems like each game (skyrim, oblivion, fallout 3, fallout new vegas) craps out at a specific amount of memory used for game saves fallout 3 i remember being so bad that i couldn't hit level 30 with 2 games saves without constant crashes (i had all the add ons and that seemed to make it a bit worse). Thats a fix for pretty much everything that can go wrong I hope this was helpful.

Also try to update the drivers that control your PC.

The easiest way to get an instructional booklet for Crash Tag Team Racing is to take your copy into a Gamestop and ask to switch it with another copy that has the booklet with it. Gamestop is very laid back and should have any issue doing this for you.

check the default diagnostic for it in ps3 itself and you will find the menu for calibrating it

Sounds like a fault with the card, but you should start by cleaning the fan first. If the problem continues and your card is under warranty then send it away to get fixed. If not under warranty, then I would look at replacing the card altogether. The amount of time you will spend going back and forth trying to find the cause may not be worth it.

maybe the is not installed correctly try to reinstall it or if the is pirated than it is corrupted

Try cleaning it with a micro-fiber cloth, I bought one for about $3.00 at my video rental store, if it's scratched get it buffed at BlockBuster don't remember how much they charge, but less then $4.00, if all that don't work, on the instruction manual is the address of the company, send it just the disk, with the sales receipt, and a letter explaning that it don't work, this happened to me with Crash Tag Team Racing, and it was BRAND new! no scratches! frooze up after passing a few levels, so I sent it back to the company, to get it replaced!
