I would guess misplaced wire.
Depending on how many and which wires you displaced to accomplish the
bulb replacement. I would have to guess that if you did everything
correctly and perfectly then the circuit board failed.
My other guess is that if you removed the front panel to circuit board
connectors, you may not have reconnected the smaller thin ones on the
left in the correct position.
If those check correct and all the other wires are in the correct
locations, then there is a repair outfit located through the internet
that seems reasonable for approaching for a more involved repair job.
The one I looked up had rebuilds available, so you might be able to get
a replacement for a better price with your old one exchanged.

The fan is in a sealed ball-bearing housing, so there's no way to oil it.
If the fan is burned out, it will need to be replaced. You can call Vollara (the new name for EcoQuest) customer service 800-989-2299 and they'll tell you what to do. If the machine is still under warranty, there will be no charge except for shipping it to the company.

I have seen it a couple of times where the cable was not properly seated into the back of the machine. If you wiggle the connector, does that make any difference? If this works, then call EcoQuest for an RMA. Is the machine still under warranty? There is a 3 year warranty on FreshAir units, if purchased through an official dealer.
I have not experienced it but I know that if you bought your FreshAir from a dealer with EcoQuest you are entitled to a 3 year warranty. Contact the manufacturer EcoQuest at 1-800-989-2299 and choose technical assistance from the menu. You will be guided through some trouble shooting and if needed you will get an RMA number so you can send your unit in for repair.
If you are not in contact with your EcoQuest dealer anymore and the unit has never been registered with the company you need a purchase receipt with the date on it.
UV bulbs, or rather the RCI cell needs to be changed every three years in the FreshAir, or when the "Replace RCI cell" appears, whichever comes first. The UV bulb loses its intensity over this period of time.
