The unit is being manually managed. You need to un-check "Manually Manage". See http://support.apple.com/kb/HT1351

Frozen Screen I have a frozen screen which says "Disk Mode", then there is a large "tick" in the centre of the screen, and another message saying "OK to disconnect". Nothing I try unfreezes this - I have...

I do not know which ipod you have, but here are instructions on how to get a nano in and out of DISK MODE:

To turn iPod Disk mode off:
  1. Toggle the hold switch on and off
  2. Press and Hold the Select and the Menu button for more than ten seconds
  3. The normal selection screen should appear, your iPod is out of Disk mode.
To turn iPod Disk mode on:
  1. Toggle the hold switch on and off
  2. Press and Hold the Select and the Menu button for six+ seconds
  3. Release both buttons when the Apple logo appears
  4. Press and Hold the Select and Play button until the Disk Mode text appears.
If it does not work, you may need to reset your ipod. The following link has extensive instructions on that procedure for many types of ipods.
Good luck my friend!

Problem with IPod Nano Model A1137

Try holding the menu button and center button at the same time. this should reset the ipod completely. Its a force power up and power down similar to Zapping your PRAM on a computer. this should reset the configurations
Hopefully that helps
