Welcome to Fixya. The PUC (PUK) is a SIM-specific code assigned by the service provider. It is near-impossible to guess and enter it; thus, if a user does not know his or her PUC and is prompted for it, it should be obtained from the service provider. You need to call T-Mobile so that they will be able to provide you with the PUK code. Thanks
You need to contact the service provider for the PUK code usually a 3 min phonecall and there is no charge for the service apart from the cost of the call
Our state PUC has a set of EAS (Extended Area Service) exchange maps. I don't see one for PA, but maybe you can search for a Norristown directory. Here are the local exchanges and a link to the PA PSC.
