Only the sim is blocked, not the phone. The unblocking code is located on the sim casing when you bought it. It will ask you to enter the code and new PIN code 2 times but will completely block it if the limited number of attempts is reached. You can just simply replace the sim and the phone will work again normally.
Complete information : The SIM card protects
your subscription and not the phone from unauthorised users. If you
change SIM cards you can still use your phone with a new SIM card. Most
SIM cards are blocked at the time of purchase. If your SIM card lock is
on you have to enter your PIN (Personal Identity Card) every time you turn on the phone.
If you enter the wrong password three times in a row the SIM card gets blocked. This is indicated by the message PIN Blocked To unblock this you need to enter your PUK

If the Message Codes do not match appears when you edit your PIN you entered the new PIN incorrectly.

To unblock your SIM card:
When PIN Blocked appears enter your PUK > OK
Enter a new four to eight PIN > OK
Re-enter the new PIN to confirm > OK
To edit your PIN:
> Settings > the General tab > Security > Locks > SIM Protection > Change PIN
Enter your PIN > OK
Enter a new four to eight digit PIN to confirm > OK
To turn the SIM card On /Off
> Settings > the General tab > Security > Locks > SIM Protection > Protection and select On or Off
if you enter the wrong pin code for 3 times your sim will get blocked ... it will ask for the puk code ... then
you must call to the customer care of your network provider and you
must tell them that your pin got blocked you want the puk code ... they may ask for the details of your sim card be sure to tell it ..
then they will provide the puk code note it down and enter into it .. your sim will get unlocked...
note : if
you enter the wrong puk code for more than 3 or 10 times the sim will
get fully blocked if then you must contact to network provider
