Got the same trouble on my gigabeat, and I think I know where trouble come from. The remote can be loose and when the connection is lost with the player or headphone, a pause fonction in engaged even if hold is on. I don't have find any way to disable it through settings but when I pluggued new headphone without the remote it never happened to pause again, so this may be the solution.
Lynda having trolled through countless message boards it seems that there are many with the same problem but no way to properly fix it!! i think i will be giving toshiba a call tomorrow to see what they have to say.
