SOURCE: Scanner excludes front page
This is not the only solution, but this might help.
Put a blank page in as the first page and see if all of your documents scan completely.
You may need to clean the ADF. Try the following:
Cleaning the Automatic Document Feeder
You may need to clean the ADF feed mechanism if your
documents do not feed easily into the ADF, or several feed in at the
same time.
To clean the ADF:
1. Dampen a soft cloth with some isopropyl rubbing alcohol
(95%).
2. Press the ADF cover release on the top left of the scanner body
to open the Automatic Document Feeder cover.
3. Wipe the feed roller from side to side. Rotate the roller to clean
its entire surface.
4. Wipe the pad from top to bottom. Be careful not to snag the
cloth on the springs on either side of the pad.
5. Close the cover when you’re finished.
NOTE: Please be careful when cleaning the roller. The roller is
designed to rotate in one direction only. As you’re cleaning the
roller, make sure it rotates easily, and continue rotating it in that
direction while wiping it clean. Do NOT force the roller to rotate
in the opposite direction. Forcing the roller to rotate will damage
it, and may cause the scanner to feed pages incorrectly.
Please let me know if this has helped and thank you. Binary Bob

Hello.
I am a Support Engineer with Xerox Scanners.
Vivek is on the right track here. The problem is likely either software or communication.
You can download the latest drivers from our website here. I have also written some instructions on the diver update process here.
As I mentioned above, the problem could also occur in the communication between the application and the scanning hardware during the transfer of the image data.
To test where this problem is rooted, you can:
*Try scanning from another application, like OneTouch. It could be some corruption in the scanning application, in which case OneTouch would operate normally. Simply repair or reinstall application to resolve.
*Check the USB connections. Try another cable (6' is standard) or another port on the back of your PC. You might also try disconnecting other USB devices while testing in case there is a conflict.
If you continue to experience trouble, please feel free to contact us by calling 800-648-0410.

If scanner is stopping @ 98% is normally due t oa communication problem. Make sure USB cable is conencted to rear of PC ,and cable is no longer than 6ft..
