In the event that your phone requests a PUK code, it indicates that you have input the SIM card PIN incorrectly more than once, locking the SIM card. Check the packaging of your SIM card, get in touch with customer service at your mobile provider, access your online account, send a text message or use a USSD code that your provider has provided, in order to find the PUK code. Your SIM card may become permanently blocked if the PUK code is entered incorrectly.

PUK lock (Personal Unlocking Key). It's a security feature on most mobile devices that protects your SIM card data and unique for SIM card.
May I suggest that you contact your sim network provider as they are the only one who can provide you the PUK codes. Once you done the PUK unlocking you can use the unlock code.
If you have secured your sim card with pin number you need to enter your pin every time you switch on your phone and if you enter wrong pin 3 times your sim get locked to unlock your sim you need "puk" code it stands for "pin unlock key"
Please call the costumer care of your sim card network operator (service provider). They will give you the number, note that if you enter random numbers several times, it'll damage the sim card and will not be usable anymore, So i suggest you not to do anything with it till you get the number from the Service Provider. That is the only way to unlock the PUK,Just Call and tell them you need the PUK code and read the number off the sim card to the customer care person when they ask for it.

your subscription and not the phone from unauthorised users. If you
change SIM cards you can still use your phone with a new SIM card. Most
SIM cards are blocked at the time of purchase. If your SIM card lock is
on you have to enter your PIN (Personal Identity Card) every time you turn on the phone.
If you enter the wrong password three times in a row the SIM card gets blocked. This is indicated by the message PIN Blocked To unblock this you need to enter your PUK

If the Message Codes do not match appears when you edit your PIN you entered the new PIN incorrectly.
If the message Wrong PIN followed by Old PIN appears then it means that you have entered the old PIN incorrectly.
To unblock your SIM card:
When PIN Blocked appears enter your PUK > OK
Enter a new four to eight PIN > OK
Re-enter the new PIN to confirm > OK
To edit your PIN:
> Settings > the General tab > Security > Locks > SIM Protection > Change PIN
Enter your PIN > OK
Enter a new four to eight digit PIN to confirm > OK
To turn the SIM card On /Off
> Settings > the General tab > Security > Locks > SIM Protection > Protection and select On or Off
