The system has had a good clean out but we still get horrible sewage smells from the sink and the toilet.

hello, this is a standard toilet you will need to inspect the flapper that is the rubber flap that holds the water up in the tank it works when you pull the lever it pulls the chain upward and lets the water flow if this flapper is old it may not stay up long enough for the water to flow. open the tank top and flush the toilet see if this flapper is staying up long enough and giving the tank enough time to empty if not change this rubber flapper its inexpensive part.
if this is not the reason then you need to adjust your water filler valve there should be a small screw on top of the pivoting float arm
if you have to adjust it for the water limit line inside the tank this will be the proper amount of water needed to flush once completely
also check that the chain lenth on the pull lever is not pulling the flapper down quicker than its supposed to,

If this is a new development, then you have a partially clogged drain or maybe a crack in the bowl assembly. If the toilet always did this, then perhaps the drain pipe was not installed correctly.
I would remove the toilet and run a snake through to the main stack. If you find that is clear, check the bowl for cracks and check the tank water level to be sure that the tank is filling to the water line on the tank

for the upstairs if the valve is fully on there is not much you can do other than replace water service to the house or heaven forbid repipe.Valves usually don't restrict fill volume.But you can take the supply loose from the tank and stick it in a bucket turn the valve on and check the flow.this should narrow it down.Down stairs repair or replace flush valve

Check your flapper line to the flush handle. The flapper is in the tank on bottom. There shoiuld be a plastic line to handle or chain. :)
